Before filling out our online registration form, please be sure of the following:

• you have a stable internet connection

• you have a WORKING personal email, preferably Gmail

• you have a hi-res softcopy file of your original PSA-issued BIRTH CERTIFICATE (old birth certificates and baptismal certificates will be accepted TEMPORARILY)

• you have a hi-res softcopy file of your ID photo, passport size, with plain white or light colored background and name tag at the bottom frame

• file attachments should be in jpg, png or pdf formats

 

PLEASE BE VERY CAREFUL AND THOROUGH IN FILLING OUT THE FORMS. READ ALL INSTRUCTIONS CAREFULLY.

Most registration problems are caused by misspellings and wrong data.  The personal data you will input should ALWAYS be based on the OFFICIAL DOCUMENTS you will submit to us.

​The prospective ALS LEARNER should be the one to fill out the form. If the learner is too young, a parent or guardian may fill out the form but have the learner beside so he/she can learn how to fill out forms. Form fulfillment is part of ALS learning.
